We are committed to providing you the best possible care. In order to keep our rates as low as possible, Crissman Family Practice has adopted the following financial policy. Please read and familiarize yourself with this policy so that future misunderstandings regarding our billing and payment policy can be avoided. If you have any questions, please do not hesitate to speak with our office. We participate in the following insurance plans. All co-payments or deductibles will be collected at the time of service. Aetna Blue Cross Blue Shield Cigna Great West Humana Medcost Medicaid Medicare Penn Western Pyramid Life Insurance Unicare United HealthCare (UHC includes: Evercare, Secure Horizion, & AARP) Wellpath If your insurance company is not listed, don't worry. It may be listed under a larger company. There are many smaller commercial insurance companies that fall under larger umbrella companies like Medcost. You are personally responsible for your charges until we receive a copy of your insurance card. If your plan does not appear above, we will file and collect the co-payments or deductibles at the time of service as a courtesy to you. If we have not heard from your insurance company after 60 days, the bill is due and payment by you is expected immediately. Please note that a surcharge may be applied for missed appointments and for co-payments not paid on the date of service. We will file motor vehicle accident reports for existing patients only, as long as we have the necessary claim filing information. Checks returned for insufficient funds must be paid in full within 10 days or are turned over to the Alamance County District Attorney's Office and subject to applicable fees. This payment must be made in the form of cash, Visa or MasterCard. Past due accounts are subject to attorney's fees, court costs, and other costs of collection. Please remember: Your insurance is a contract between you, your employer, and your insurance company. You are personally responsible for any bill, or portion thereof, not paid by your insurance company.
